Appointments
New Patients: If you are a new patient, please arrive 15 minutes prior to your scheduled appointment. At your first visit, please provide the following information:
- Please Complete Patient Registration Packet
- Please Complete Medical History Form
- Present your insurance card and license
Established Patients: If you are a returning patient, please arrive 10 minutes prior to your scheduled appointment. At your return visit, our staff will confirm your information to ensure our records are accurate.
For urgent medical matters, please call us at (978) 459-2152. If you have a true medical emergency, call 9-1-1.
Routine Adult Physicals
We encourage our patients to plan in advance for routine annual exams, as these can take several weeks to schedule. Please note that most insurance companies allow one physical per calendar year for adults.
